School Year CNA Program at DeForest - 16 spots available each year
The following must be completed in order to be eligible for this course:
1. Create a Madison College Student Account (if you do not already have one through Med Terms course or others). You can do this at the Madison College Website.
(After creating your student account, activate your myMadisonCollege login; this gives you access to your Madison College email, Student Center and Blackboard accounts.)
2. Demonstrate Reading Comprehension Requirement by ONE of the following:
- Placement Testing (official scores from one of the following, within the last two years):
- ACT Asprire Reading Score (428+)
- Pre ACT Reading Score (22+)
- ACT Reading Score (13+)

3. Background Check and Health Screening
NOTE: depending on your vaccination history, you may need updated immunizations or titers (blood draws) to demonstrate your immunity. IMPORTANT: Tuberculosis (TB) testing -- you will need at least 10 days between the two TB tests.
- Scan, save and upload your completed Background Information Disclosure (BID) and Immunization and TB record to your CastleBranch account; be sure to include any supporting lab reports/documentation. For assistance please see Ms. Buenning.
- Health/accident insurance is required to take this course. Discuss this with your parents.
- After uploading the required documents, be sure to check your email account for any 'to do' messages from CastleBranch. Read your background check results carefully.
- When all requirements in your CastleBranch account are marked complete, you will receive a message in your Madison College account notifying you are approved to register for your CNA Course. If you receive a rejection on your Influenza immunization, please email Ms. Buenning.
Other Important Information
- Log in to the Blackboard course site at least three days prior to your first day of class to familiarize yourself
- You are not allowed to miss any class, lab or clinical time. Do not take this course if you expect ANY absences.
- Solid-color scrubs and clean impermeable shoes are required for lab and clinical.
- If you need to drop the course, you must do so before the first day to receive a full refund.
- You must be at least 16 years of age prior to the first day of class.
