The DeForest Area School District offers employees Group Health Cooperative (GHC) Health Insurance. Employees can choose between GHC Primary Care Preferred $0 Deductible/$1,000 MOOP HMO Plan or the Point-of-Service (POS) Plan. Employees who reside outside of Dane County can also choose the Preferred Provider Option (PPO).
The HMO provides coverage for all in-network providers, out-of-network coverage only with prior authorization, and emergency or urgent care services anywhere. POS provides coverage for in-network providers, plus provides coverage when the member chooses to seek health services from out-of-network providers. With the POS, out of network services are paid at different benefit coverage levels. The information below is a brief overview of the coverage provided. Employees regularly scheduled to work at least 30 hours per week are eligible to participate in one of the District health insurance plans.
