- Premium: 100% Paid by District
- Eligible Employees: Employees regularly scheduled to work at least 20 hours per week
- Effective Date of Coverage: 1st day of Month following Hire Date
- Elimination Period: 60 days of continuous disability
- % of Covered Salary Insured: 90% of monthly salary up to maximum benefit
- Insurance Carrier: Madison National Life Insurance Company
