Short Term Disability

The DeForest Area School District offers employees regularly scheduled to work at least 20 hours per week the opportunity to enroll in a voluntary Short Term Disability plan.  Coverage pays an elected weekly benefit when unable to work as a result of injury or illness. 

• Premium: 100% Paid by Employee and based on amount of coverage approved by carrier. 
• Enrollment: Guaranteed up to $301 weekly benefit without medical underwriting if enrollment is submitted within 30 days of hire date.
• Weekly Benefit: Cannot exceed 66-2/3% of the employee's annual salary.
• Insurance Carrier: Madison National Life Insurance Company

To apply for coverage or to file a claim for Short Term Disability benefits, contact the Human Resources Department. Evidence of insurability questionnaire will need to be submitted for late enrollees, requests for coverage increases and enrollment amounts exceeding the $301 guarantee issue.
